TERMS AND CONDITIONS: This is a contract for hire for limousine services at a per hour rate, including vehicle and chauffeur. A 15% gratuity will be added to the hour contract rate as part of this agreement. Payment of unpaid balances are authorized by signed acceptance of this contract. I accept full responsibility and agree to pay a minimum of $200 for any repair and/or replacement of any damage to this vehicle, and/or general cleaning that exceeds usual use or wear while in my or my guest's possession. Cigarette burns, torn seats, and bodily fluids are subject to a $500 charge, which will be billed to the cardholder reserving the limousine. The cardholder will be billed for any broken or stolen glasses at a cost of $15 per glass. DRUGS OF ANY KIND ARE STRICTLY PROHIBITED. PERSONS UNDER 21 YEARS OF AGE ARE PROHIBITED FROM CONSUMING ALCOHOLIC BEVERAGES WHILE THE LIMOUSINE IS FOR HIRE. Misconduct, drug and alcohol violations are grounds for early termination without refund at the discretion of the chauffeur. All reservations require a 1/3 deposit, non-refundable. Cancellation within five (5) business days of the reserved time will forfeit the total amount. In the event financial responsibility is not met, and attorney and/or collection agency services are needed, the customer and/or card holder agrees to pay all fees. Eclipse Limousine is not responsible for items left in vehicle at any time.
